Rêve Health, Inc. is a health and wellness coaching and education company that focuses on the promotion and maintenance of health and wellness.
Rêve Health, Inc. services (hereinafter “program” or “services”) may include education and consultation on sleep, nutrition, stress and lifestyle management, exercise, and recommendations for labs to fully identify one’s own individualized needs to reach optimal health, physical outcomes, and quality of life. We are not medical providers and do not provide medical advice.
Health is not just the absence of disease, but the energy and vitality to live and perform at the highest level possible.
To receive our services, you understand and agree to the following:
● You must be 18 years older to receive our services or have the consent of a parent or guardian.
● Payment is required in advance or at the time of service. Insurance does not cover our services. Therefore, you will not be able to submit claims to your health plan for reimbursement.
● Our program/services may be delivered in-person or online, and in an individual or group session.
● Our health and wellness coaching and education programs/services are not the practice of
medicine and do not substitute for medical or physical exams or consultations. If you have
medical or health concerns about symptoms, pain, impairments or disabilities, you should seek a
consultation from your PCP or other appropriate medical provider. We reserve the right to
discontinue our services to you if we believe at any point in the delivery of our coaching services
that your symptoms or complaints require a medical examination.
● We recommend you get medical clearance from your primary care provider (“PCP”) before
participating in our program/services. Your participation in our program/services is solely at your
own risk whether you obtain medical clearance or not. You knowingly and freely assume all such
risks, both known and unknown, and assume full responsibility for your participation.
● You consent to us communicating our recommendations, questions and concerns about your
health care with your PCP and other treating health care providers as appropriate. We may request
that you sign a HIPAA Authorization to allow your health care provider to communicate with us
as well.
● We may recommend that you obtain and perform certain lab tests on your own or obtain them
from another qualified medical provider. The purpose of such functional medicine laboratory tests
is to evaluate nutritional, biochemical, or physiological imbalance and to identify any need for a
medical referral. These lab tests are optional and are not intended to diagnose, treat, prevent or
cure disease and your participation in this testing is at your discretion and personal cost. As such,
our recommendation that you perform over-the-counter self-tests or recommendation to obtain lab
tests from another qualified medical provider does not constitute the practice of medicine. You
are responsible for the proper administration of any lab tests you choose to take in accordance
with test instructions.
● We may provide general nutritional education and make individualized recommendations
regarding use of supplements and herbs in order to promote homeostasis in your diet and to
supply nutrition to support the physiological and biochemical processes of the human body. Our
general nutrition education and recommendations regarding vitamins, supplements and/or herbs
does not constitute the practice of medicine or medical nutritional counseling. Please consult a
licensed dietician or your physician if you need specific nutritional advice for a medical problem.
● You understand that vitamins, minerals and trace elements, amino acids, herbs, or homeopathic
remedies are not classified as drugs and are therefore not regulated by the Food and Drug
Administration. However, these substances can have significant effects on physiology and must
be used rationally. The use of nutritional supplements may be safely recommended for individuals
who are using pharmaceutical medications (drugs), but some potentially harmful interactions may
occur. You are responsible for notifying your primary care provider and other treating
physicians about the supplements and herbs we have recommended to ensure there is no risk of
an adverse interaction with other prescribed drugs or other concerns. We may require you to
obtain medical clearance from your primary care provider before we provide recommendations
on nutritional supplements or herbs if you are currently taking prescription medication and/or if
we have any concerns about potential drug interactions.
● If any of our coaching sessions occur on-line, you acknowledge and accept the potential
technology risks of participating in an online program, including but not limited to the risk of (1)
interruption of the audio/video link, (2) disconnection of the audio/video link, (3) video that may
not be clear, and (4) potential of unauthorized access to the live or recorded session. We are not
responsible for these or other technology problems.
● You understand and agree that we have made no guarantees or assurances of any particular
outcome from our health and wellness coaching program/services or recommendations.
● You agree, on behalf of yourself and your heirs, assigns, personal representatives and next of kin,
to assume these and all injury risks and waive all liability against Rêve Health, Inc. and
its officers, members, employees, subcontractors, agents, assigns and
other participants and sponsoring agencies (“Releasees”) for any and all claims, lawsuits,
damages, liability, costs and expenses, including reasonable attorneys’ fees, for any personal
injury or personal property damage claims, disability, death, or loss or damage to person or
property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent
permitted by law.
● This Agreement shall be governed by the laws of the State of Massachusetts.
